*  Complete the medical release form attached below and email to
    [email protected].  
*  If NEW to our league, provide a copy of child's birth certificate and an updated picture.

 ONLINE APPLICATION & DOCUMENTS REQUIRED

*  The fee covers the cost of your child’s uniform, team playing equipment, fees for umpires as well as a trophy (if child plays 75% of games).  All children must have their own gloves/helmet.
